If you wish to seek reimbursement for out-of-network services through your insurance provider, we can provide a Reimbursement Invoice (aka Superbill) upon request.
This document will include:
ICD-10 and CPT codes (diagnosis and procedure codes)
Provider’s name, credentials, and NPI number
Tax ID and clinic information
Dates of service and payment details
You are solely responsible for submitting this invoice to your insurance company. Reimbursement is not guaranteed, and Amatoria Wellness does not communicate with or follow up with insurance companies on your behalf.
We’re happy to support you with accurate documentation, but it is your responsibility to verify your benefits, submit any claims, and handle appeals directly with your insurer.
